Looking at this after a few days around other stuff, it seems
refreshingly clean.

One point, e.g. the example in 3.4.3.2, where "The formatting of the
content is determined by the categorization schema chosen.". Shouldn't
there be some nearby reference to that schema? An element stating the
schema URI, maybe? (value="01001000" seems a little lost on its own)

Yep, I think you're right. There's enough distraction with URLs
staggering in off the street.
