Just a thought - might it be feasible to let the content element itself act
as a pseudo root :

<content type="application/xhtml+xml" mode="xml"
xmlns="http://ns-for-xhtml";>
Hello <b>world</b>!
</content> ?

or even, as the mimetype is declaring this to be xhtml, might it simplify
things further to optionally leave out the xmlns and default to (say) the
xhtml-basic profile?
