Out of habit, I went and edited the wiki with a few comments rather than
sending them here. (oops)  My name is by each section I changed, but here
is a quick summary:

* why ERR instead of ERROR?
* use a request body rather than headers
* your 501 (Method Not Implemented) example is exactly what I would
  expect, given the Allow: header that the server sent (which is also a
  good way for a client to determine whether reporting is _possible_)
