SET questions.

Hello, everybody.
Let me ask some questions about SET. I want to have clear concept on SET.

1) Cardholder,Merchant,Issuer,Acquirer,Payment Gateway,CA are the 
components of SET.
I am not sure who are Issuer ,Acquirer and Payment Gateway. Is Issuer the 
bank which have the account of Cardholder ? And is Acquirer the bank which 
have the account of Merchant ?
Card company like Visa or Master correspond to what - CA or Payment Gateway 
or others ?
Please give me the simple example which includes the function of each 
entity and who they are.

2) Merchant confirm the order Cardholder sent and request the money of 
product to Acquirer or other (I am not sure) .
After that, Cardholder will receive the bill from a card company and there 
will be any fund transfer from Cardholder to a card company. Is this 
consecutive procedures - underlined sentence- included in SET boundary ?
