UW imapd can send BYE outside of a command. Do you have an example of OK outside of a command? I can't think of any.

Don't you send something like '* OK NAT middleboxes must die!" (more politely phrased) every two minutes, or am I confusing you with someone else?

That's only when IDLE is running.
