Why is an LDUP response code necessary?

First, the LDUP response codes listed don't cover all possible
responses.  noSuchObject or invalidDNSyntax are just a few.

Second, it is completely redundant, since the extended
response already includes a response code.  This is confusing
- how is an invalid DN in a startReplication reported back?

I would propose eliminating it entirely, and allocating one of
the available codes for any additional codes LDUP needs.
