Trevor, I have one additional comment.

Section 4.9 responder: This item is not adequately described in this
section, nor is it mentioned anywhere else in the specification.  I
presume the value of this field is the value that the SCVP server
stuffs into the requestorRef to identify itself when relaying
requests.  But if the value is "only of local signifigance to the SCVP
server" then why is it returned to the client in the response?  What
could the client possibly do with information?
