[IETF-PKIX] PKIX part 1: faulty ASN.1:1994

I found some problems in the 1994 ASN.1 in Appendix B.

- UniqueIdentifier is *not* defined. It's a simple definition, but without
it the module should not compile. This is a bug and needs to be fixed.

- AttributeValueAssertion is defined but never used (and didn't appear in
the 1988 ASN.1). This shouldn't be a problem, but should be fixed before
the draft moves on.

- The names issuerUniqueIdentifier and subjectUniqueIdentifer were called
issuerUniqueID and subjectUniqueID in the 1988 version.  This shouldn't be
a problem, but should be fixed before the draft moves on..
