Just a note that I think we should do a section that will outlines the
duties of each agent. Ie that injecting-agents should add originator-info
and message-id etc and that relayers MUST not change anything except the
Path.
This should remove extra bits from the definitions of each header so that
for References we just need to specify the syntax rather than what what a
followup-agent should shorten it (if it must do this).
