I believe that's a deviation from RFC 2234 which is not necessary
and does not add to the clarity of the specification.
Note that we also do need a large set of rules to define each Foo-
Name, Foo-content, Foo-parameter. Another rule that defines the
header does not hurt to much and makes the spec much easier to
read.
