Our goals may be quite similar...

The right tool for the right job. A XML DTD specific for
describing *single* individuals can be used in conjunction with
other DTDs that are specific to their tasks, such as geneology. I
think trying to force Vcard-XML to be used for other purposes
would be a mistake, especially as there are easier ways to do it
via Xlink or XML namespaces.
Create a DTD to describe the relationship between individuals,
but hold the individuals data in Vcard-XML, then reference that
data through XLink.

The only occurrence that I can think of  where you may require
multiple instances of a familly name is for Spanish speaking countries
where an individual's surnames is composed of their mother's maiden
name and father's surname. Here it could be argued its necessary
to separate the two surnames otherwise surnames concatenate through
generations.
