Microsoft Exchange Server is a product that only maps between Content-Type and 
file extension.

If using something other than application/octet-stream for the second part 
doesn't break anything, then we should do it, and I think this was the 
consensus.

Ned Freed or Jim Galvin would be the best people to speak about the use of 
application/octet-stream for the second part, and whether or not the decision 
was arbitrary enough to change it.
